Our mission is to enhance the dignity and quality of life through nutrition and supportive services.

Many of the seniors we serve in Sacramento are isolated and lonely as well as financially insecure. We address these challenges by providing nutritious home-delivered meals, helping to decrease food insecurity and nourish the whole person.

Seniors Caseworkers assesses for possible enrollment

Our Caseworker team provides

HOME VISITS:

Our Meals on Wheels Sacramento caseworkers will visit all new seniors in their homes, to determine eligibility. Caseworkers will connect these Seniors with programs in the community that will help them age in place and maintain their independence. “

Caseworkers will then check in with our participants on a quarterly basis to assess their needs and satisfaction with our services.

ONGOING SUPPORT:

Meals on Wheels Sacramento does more than just deliver nutritious meals. We also provide human contact, and welfare checks if needed.

We stay in close contact with our participantsand drivers throughout the year to make sure that they have access to all the services that they need. We can also provide some supportive devices (when funding allows), and programs to make home life easier.

The Friendship Line is both a crisis intervention hotline and a warmline (non-urgent calls); our services provide older and disabled adults reassurance, crisis intervention, information and referrals.
